• Please arrive 10–20 minutes early. Failure to do so may require your appointment to be rescheduled.
• Bring a valid Texas photo ID.
• Carry all active insurance identification cards.
• Bring an updated list of all current medications.
• Provide your most recent Primary Care Physician visit note and latest laboratory results.

Copayments, deductibles, and coinsurance are collected at check-in. If you have questions regarding your coverage or expected payment, our staff will gladly assist you before your appointment.

Please notify our office at least 24 hours in advance if you need to cancel or reschedule. Missed appointments or late cancellations may be subject to applicable fees.

Please ensure your contact details, medications, and medical history are current. Arriving late may require your visit to be rescheduled, depending on physician availability.

• Sign the documents during check-in with our staff.
• If you are unable to complete the paperwork beforehand, please arrive at least 1 hour early.

• Complete every required field.
• Do not abbreviate your name.
• Enter your email address when prompted.
• Verify your email using the Adobe confirmation message.
• You do not need to download or email the completed forms—we receive them automatically after verification.
